The wellness industry’s answer — test everything, the longer the better — is the opposite mistake, buying false positives and anxiety more than insight. The right approach is in between: a deliberate panel, built in tiers, that covers what matters and skips what doesn’t. Here’s how a pharmacist would assemble it.


What the “Standard” Physical Actually Covers

A typical physical orders some mix of a complete blood count (CBC), a comprehensive metabolic panel (CMP), and a lipid panel — sometimes a TSHTSH (thyroid-stimulating hormone) A pituitary hormone measured to check how well the thyroid is working.. Tier 1: The Foundation (Worth It for Almost Everyone)

These are the broadly useful, inexpensive, widely available tests that belong on nearly everyone’s annual draw.

  • Complete blood count (CBC). Screens for anemia, signs of infection or inflammation, and blood-cell abnormalities.
  • Comprehensive metabolic panel (CMP). Kidney function (creatinine, eGFReGFR (estimated glomerular filtration rate) A calculation from a blood creatinine test that estimates how well the kidneys are filtering waste. A declining eGFR is the primary indicator of worsening kidney function.), liver enzymes (ALT, AST), electrolytes, and fasting glucose. The liver enzymes are quietly important — they’re often the first hint of fatty liver disease (MASLD), though a normal ALT doesn’t rule it out.
  • Lipid panel. Total cholesterol, LDL, HDL, triglycerides — the baseline cardiovascular screen, best interpreted against risk-based targets rather than the lab’s “normal” flag.
  • HbA1cHbA1c (hemoglobin A1c) A blood test that shows your average blood sugar over the past ~3 months. plus fasting glucose. HbA1c reflects roughly three months of average blood sugar. The US Preventive Services Task Force recommends screening adults 35–70 who carry extra weight, and the diagnostic thresholds are worth knowing: fasting glucose under 100 mg/dL is normal, 100–125 is prediabetes; HbA1c of 5.7–6.4% is prediabetes, 6.5% or higher is diabetes.
  • TSH. A reasonable thyroid screen, particularly with relevant symptoms or a family history.

This foundation is cheap, standardized, and covers a lot of ground. For many healthy people it’s most of what they need — provided it’s read correctly.


Tier 2: The High-Value Additions Most Physicals Skip

This is where a generic checkup becomes a real risk assessment. None of these is exotic; they’re routine in preventive cardiology and metabolic medicine, yet they rarely appear on a standard physical.

  • ApoB. A direct count of atherogenic (artery-clogging) particles, and a better predictor of cardiovascular risk than LDL cholesterol alone. If you add one cardiac marker beyond the standard panel, this is the one. See ApoB versus LDL.
  • Lp(a). A largely genetic, independent cardiovascular risk factor that standard panels ignore. Society recommends measuring it at least once in adulthood — it raises risk even when LDL is controlled, and most people with high levels have no idea.   • Fasting insulin (for HOMA-IRHOMA-IR A simple score from fasting glucose and insulin that estimates how insulin-resistant you are.). Paired with fasting glucose, fasting insulin lets you calculate HOMA-IR, an estimate of insulin resistance that often turns abnormal years before glucose does. It’s one of the earliest windows into metabolic trouble.
  • hs-CRPCRP (C-reactive protein) A blood marker of inflammation made by the liver. The high-sensitivity version (hs-CRP) is used for heart-risk assessment.. High-sensitivity C-reactive protein, a marker of low-grade inflammation. The CDC/AHA framework reads it in risk tiers — under 1 mg/L lower risk, 1–3 average, above 3 higher — making it a risk-stratification tool rather than a simple pass/fail.   • Vitamin D (25-OH). Reasonable to check if you have a reason to be low, with the honest catch that the “right” target is genuinely debated and has shifted over time — the Endocrine Society now advises against routine testing in healthy adults. Useful for identifying real deficiency; not a number to obsess over.
  • Ferritin and iron studies. Worth adding for menstruating women, anyone with fatigue, or those on plant-heavy diets — a wide “normal” range can hide low-iron states at the bottom and inflammatory or metabolic signals at the top.

Add ApoB, Lp(a), fasting insulin, and hs-CRP to the foundation and you’ve covered the cardiometabolic story far better than a standard physical does — for a modest extra cost.


How Often to Test What

Selection is half the job; frequency is the other half.

  • Annually for most of the foundation and cardiometabolic additions — lipids/ApoB, HbA1c, fasting glucose and insulin, hs-CRP, CBC, CMP — enough to see a trend without over-testing.
  • Once (usually for life) for Lp(a), since it’s genetically determined and stable.
  • On a shorter interval for anything you’re actively trying to move — rechecking LDL/ApoB or HbA1c a few months after a diet, exercise, or medication change.
  • Less often for stable, reassuring values with no symptoms or risk change; a yearly TSH in an asymptomatic person adds little.

The principle: test frequently enough to catch a trend, not so often that you’re chasing noise.


What to Skip (or Approach With Caution)

A smart panel is defined as much by what it leaves out. For healthy, average-risk people, these are generally low-yield:

  • IgG “food sensitivity” panels. Not validated to diagnose food intolerance — major allergy organizations advise against them. A positive IgG usually just reflects exposure, and acting on it leads to needless dietary restriction.
  • Broad, untargeted micronutrient screens. Testing every vitamin with no specific indication mostly surfaces incidental abnormalities that don’t change management. Test for a deficiency when there’s a reason to suspect one.
  • Routine cancer-marker blood tests (CA-125, CEA) in asymptomatic, average-risk people — not recommended for general screening, since they generate false positives and rarely catch disease early.
