Tadalafil (Cialis) for Longevity: Heart, Brain, and Why Everyone Is Talking About It

Tadalafil (Cialis) is gaining serious attention as a longevity molecule. The peer-reviewed cardiovascular, mortality, and dementia evidence — and the catches that keep it from being a proven longevity drug.

If you spend any time in longevity circles — or follow Bryan Johnson — you’ve probably seen tadalafil come up in a context that has nothing to do with erectile dysfunction. Johnson takes 2.5 mg daily. Not for erections, he says, but for longevity.

The argument behind this is worth taking seriously. Erectile dysfunction is a vascular condition — an early sign that blood vessel function is breaking down. A drug that treats that underlying dysfunction might also improve long-term cardiovascular health. That logic is sound, and the data is starting to support it.

It’s not a proven longevity drug. But it may be the most compelling repurposing story in this space right now. What follows is a grounded review of the current evidence.


How It Works: The Nitric Oxide Connection

Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor. PDE5 is an enzyme that breaks down cGMP — a molecule that signals blood vessels to relax and widen. By blocking PDE5, tadalafil raises cGMP levels, which boosts nitric oxide (NO) activity in the walls of blood vessels.

Nitric oxide is one of the most important molecules for vascular health. It keeps blood vessels flexible, reduces inflammation, and prevents blood from clotting too easily. The problem is that nitric oxide availability naturally declines with age — driven by oxidative stress, chronic low-grade inflammation, and the gradual aging of blood vessel cells.

This is the core of the tadalafil-longevity argument. By consistently raising cGMP and restoring some of that nitric oxide activity, the drug may slow the vascular deterioration that underlies most cardiovascular disease. Animal studies also suggest PDE5 inhibition reduces heart muscle damage during periods of low blood flow. That’s promising — but animal data doesn’t automatically translate to humans, and this area needs clinical trial validation.


The Cardiovascular Evidence

The strongest data comes from a 2024 meta-analysis published in the European Heart Journal — Cardiovascular Pharmacotherapy. It pooled 16 studies involving 1,257,759 subjects with a median follow-up of 4.3 years. The findings:

  • Major adverse cardiovascular events (MACEMACE (major adverse cardiovascular events) A trial endpoint that bundles together heart attack, stroke, and cardiovascular death.) were 22% lower in PDE5 inhibitor users vs. controls (RR 0.78; 95% CI 0.69–0.89).
  • In men using PDE5 inhibitors specifically for erectile dysfunction, all-cause mortality was 37% lower (RR 0.63; 95% CI 0.49–0.81).

A separate 2024 observational cohort study in Clinical Cardiology by Kloner et al. focused on tadalafil specifically — not just the drug class — using insurance claims data from over 29,000 men with erectile dysfunction. Over a mean follow-up of 37 months:

  • Overall MACE: 19% lower (HR=0.81; 95% CI=0.70–0.94; p=0.007)
  • Unstable angina: 45% lower (HR=0.55; 95% CI=0.37–0.81)
  • Coronary revascularization: 31% lower (HR=0.69; 95% CI=0.52–0.90)
  • Cardiovascular-related mortality: 55% lower (HR=0.45; 95% CI=0.22–0.93)

One of the most compelling findings in the Kloner data was a clear dose-responsedose-response The pattern that links how much of a drug you take to how big an effect you get. A clear dose-response means higher doses generally produce more effect — up to a point. relationship. Men with the highest cumulative tadalafil exposure had a 60% lower rate of MACE compared to those with the lowest exposure (HR=0.40; 95% CI=0.28–0.58). In clinical research, a dose-response pattern strengthens the case for a real biological effect — it makes it harder to dismiss the results as coincidence or selection bias.


All-Cause Mortality: What the Numbers Show

The mortality data is where the longevity conversation gets the most traction — and the numbers warrant a close look.

In the Kloner 2024 study, all-cause mortality was 44% lower in men exposed to tadalafil (HR=0.56; 95% CI=0.43–0.74; p<0.001). The meta-analysis found a 30% reduction in all-cause mortality across all studies (RR 0.70; 95% CI 0.56–0.87).

A 2025 large-database study in The American Journal of Medicine by Jehle et al. used the TriNetX platform to compare men prescribed tadalafil for erectile dysfunction or an enlarged prostate against matched controls. The tadalafil group showed significantly lower rates of all-cause mortality (RR 0.66), heart attack (RR 0.73), stroke (RR 0.66), and blood clots (RR 0.79) — with tadalafil consistently outperforming sildenafil on every endpoint.

These numbers are notable. But context matters. These are all retrospective observational studies. Men who get prescribed tadalafil tend to be more engaged with their health, see doctors regularly, and may be metabolically healthier than those who don’t. That’s known as healthy user bias, and no statistical adjustment fully removes it. The signal is real. Whether it’s purely causal is still an open question.


The Dementia Question

Some sources have begun citing tadalafil as a tool for dementia prevention. The data here deserves careful handling.

The biological rationale is plausible. Better blood flow to the brain, reduced inflammation, and improved signaling between blood vessels and neurons are all possible ways PDE5 inhibition could protect the aging brain.

Epidemiologically, the Jehle 2025 study showed tadalafil was linked to a 32% lower rate of dementia (RR 0.68). A dedicated 2024 cohort study in Neurology (Adesuyan et al.) used UK health records from 269,725 men with erectile dysfunction over a median 5.1 years. PDE5 inhibitor use was associated with an 18% lower risk of Alzheimer’s disease (HR 0.82; 95% CI 0.72–0.93). A dose-dependent pattern emerged as well — men issued 21–50 prescriptions had a 44% lower risk (HR 0.56). That’s a real signal.

The important caveat: when the authors built in a 3-year delay before counting cases — to account for the early, pre-symptom stage of Alzheimer’s and reduce reverse causationreverse causation When the presumed cause and effect are backwards — the illness lowers the marker rather than the low marker causing the illness. A common trap in nutrition research. — the association weakened and was no longer statistically significant (HR 0.93; 95% CI 0.80–1.08). That shift in findings matters. It means the protective effect seen in the primary analysis may partly reflect the fact that men already in early cognitive decline are less likely to be prescribed tadalafil — not that the drug is protecting them.

A clinical trial (NCT07172815) is in development to test tadalafil directly in early Alzheimer’s disease (estimated start: late 2025). That will be far more informative than any observational data we have now.

Clinical callout: The epidemiological signal linking PDE5 inhibitor use to lower Alzheimer’s risk is consistent, and the dose-response pattern adds credibility. But the sensitivity analysis raises enough doubt about causation that calling tadalafil a dementia-prevention drug would be premature. The mechanism is plausible, the signal is real, and the trial is coming. That’s where this stands.


Daily Low-Dose Tadalafil: What That Looks Like

For context: tadalafil is already FDA-approved at 2.5 mg and 5 mg daily for both erectile dysfunction and benign prostatic hyperplasia (BPH, or an enlarged prostate). The once-daily dosing is what makes it attractive as a long-term intervention. Unlike sildenafil, which is taken as needed, tadalafil has a half-lifehalf-life The time it takes for half a drug dose to clear the body. A longer half-life means less frequent dosing. of roughly 17.5 hours. That means it maintains steady levels in the bloodstream with daily use — which likely explains the sustained effects on blood vessel function seen in the research.

The FDA-approved range for erectile dysfunction is 2.5–5 mg daily or 10–20 mg on demand. Most longevity-focused physicians discussing tadalafil are working with the 5 mg daily dose, which aligns with the dosing used in most of the relevant studies.


Important Caveats

Several clinical points deserve emphasis:

The contraindication with nitrates is absolute. Tadalafil amplifies the blood pressure-lowering effect of nitrate medications. Combining the two can cause a dangerous drop in blood pressure. Co-administration is contraindicated. If someone on tadalafil needs emergency nitrate therapy, at least 48 hours must pass after the last tadalafil dose. This is the most critical safety consideration with this drug class.

Use caution with alpha-blockers and blood pressure medications. Both can add to tadalafil’s blood pressure-lowering effects, raising the risk of dizziness or fainting.

All longevity evidence is still observational. No randomized trial has shown tadalafil extends human lifespan. The data is consistent and the biology makes sense, but consistency is not proof. What exists is a strong signal — not a confirmed outcome.

The data comes almost entirely from men. Whether these cardiovascular benefits apply to women is unknown.

Funding disclosure matters. The Kloner 2024 study received partial funding from Sanofi, a manufacturer of a tadalafil product. The methods were rigorous and findings internally consistent, but that relationship is worth knowing.

Daily tadalafil, for the right person and with a prescriber’s oversight, is one prescription lever — not a standalone longevity plan. The pharmacist's bottom line

The cardiovascular data for tadalafil is among the most consistent observational signals I've seen for any repurposed drug in the longevity space — a 22–30% reduction in mortality and cardiovascular events, with a clear dose-response relationship, across 16 studies and 1.2 million subjects, on top of a well-understood mechanism. But I want to be careful not to get ahead of the evidence: we have very compelling data that men who take tadalafil tend to have better outcomes, not randomized controlled trial (RCT) data showing that prescribing it to otherwise-healthy men for longevity reduces their risk of dying. Those are related, but not the same thing. If a male patient already has a legitimate indication — erectile dysfunction, BPH, documented endothelial dysfunction, or significant cardiovascular risk — this is a conversation worth having, because the safety profile is well-characterized and the risk-benefit looks favorable for the right patient. For men pursuing it purely off-label for longevity, I'd discuss the observational caveats, screen carefully for nitrate use and blood pressure risk, and set realistic expectations. This is not a proven longevity drug yet — but it is the most promising cardiovascular signal in this space in years. Watch for RCT data.
