Form: Monohydrate is the only form with robust clinical data. Ignore creatine HCl, ethyl ester, and other “advanced” forms — they add cost without evidence.

Safety note: Creatine causes a modest, expected rise in serum creatinine due to its metabolism — not kidney damage. If your provider flags a slightly elevated creatinine, mention you’re taking it.

Part 2: Nutrient Repletion

Vitamin B12 Strong

Dose: 1,000–2,000 IU vitamin D3 daily for most people, paired with 100–200 mcg K2 as MK-7. Higher doses are for correcting a documented deficiency, not for maintaining an already-adequate level.

Testing note: A 25-OH vitamin D level below 20 ng/mL is genuine deficiency and warrants repletion. The 30 ng/mL cutoff most consumer lab reports flag against — and the 40–60 “optimal” band quoted widely online — are conventions rather than targets validated against health outcomes.

Magnesium Glycinate Moderate

Part 3: GI Tolerance

Nausea is the most common early side effect, driven by delayed gastric emptying and direct stimulation of the area postrema (the brain’s vomiting center). Dose escalations are the highest-risk periods.

Ginger (Zingiber officinale) Moderate

Ginger has more evidence for nausea than any other supplement in this space. It works on the same 5-HT3 nausea-receptor pathway that ondansetron (Zofran) targets, helps the stomach empty more normally, and calms gut inflammation. Digestive Enzymes Low–Moderate

The evidence here is thinner. GLP‑1 medications don’t impair pancreatic enzyme production, so the rationale is weaker than for ginger. Still, delayed gastric emptying can alter the timing of food mixing with enzymes, and some users report meaningful relief — particularly with high-fat or high-protein meals. Low risk, largely anecdotal, but reasonable to trial during dose escalations.
