The short version
- The 'standard' annual physical panel — a basic metabolic panel, a blood count, and a cholesterol panel — is a reasonable floor, but it leaves out several of the markers that best predict long-term cardiometabolic risk.
- The highest-value additions most physicals skip are ApoB, Lp(a), fasting insulin (for HOMA-IR), and hs-CRP — markers a preventive cardiologist or metabolic specialist routinely orders.
- Lp(a) is largely genetic and stable for life, so it generally only needs to be measured once — a rare 'test it a single time' marker that can meaningfully change your risk picture.
- More tests is not better. Every marker carries a false-positive rate, so a giant unfocused panel almost guarantees a few meaningless flags. The goal is the right tests, ordered with intent.
- Some popular tests are low-yield or actively discouraged for healthy people — IgG 'food sensitivity' panels, broad untargeted micronutrient screens, and routine cancer-marker blood tests in average-risk adults.
- Frequency matters as much as selection: most markers are worth checking yearly, some (like Lp(a)) once, and stable values don't need constant re-testing.
